When is the optimal time to load a Dental Implant?
Remove this doubt with an implant specif MulTipeg and a Penguin RFA device.
An ISQ (implant stability coefficient) measure is possible in less than a second with PenguinRFA. When attached to the implant the MulTipeg is excited by magnetic pulses from the PenguinRFA device and vibrates. PenguinRFA measures and translates the vibrations to an ISQ scale between 1 and 99.
At placement, an implant is reliant on its mechanical stability called primary stability. This number will drop shortly after placement due to micro-trauma at the implant-bone contact caused by the drilling and compression forces. As the bone remodels around the implant biological stability forms. For this reason, two or more measures are recommended to show ISQ changes at placement during the healing period and or prior to loading.
A Penguin RFA value above 70 indicates a very stable implant with low micro-mobility.
Low trauma implant and regenerative techniques.
Penguin allows clinicians the opportunity to monitor bone healing and implant stability and understand the soonest time to load the fixture. Insight such as this removes doubt and allows loading at 12 to 14 weeks when used with modern membrane-free, form stable fully resorbing grafts such as Powerbone Dental Putty.
PenguinRFA is non-invasive, objective, accurate and repeatable.
Can stability be measured on an Abutment?
In short – Yes!
We know that disassembling prosthetic abutments during healing is a risk and can when repeated lead to marginal bone loss (1). For this reason, the ‘one-abutment-one time restorative method is often favoured with abutments such as the Bioner Minicone.
In order to apply the one-abutment–one-time concept, transducers screwed onto the abutments are available for Penguin. As the transducer is placed on top of the abutment the measurement is simpler for the clinician. However, attaching a peg to the abutment rather than the implant means the PenguinRFA device will be further away from the bone at measurement which could impact ISQ values. Work by Lages et al (2) highlights the importance of always analysing the ISQ values in the same way (either on platforms or abutments). Lages measured ISQ on permanent abutments at heights of 1-3 mm using MulTipeg item 72.
Which Penguin RFA?
There are two Penguin units available at low online prices at the Implant Store.
- Penguin RFA – The original unit.
- Penguin II – The new kid on the block.
Both units work with the same Penguin MulTipeg and driver. Penguin II adds a clearer display, a more tactile soft rubber surface and a desktop main charging station. Both units have rechargeable batteries which can be removed and replaced easily in Penguin II for longer operational life.
